What changed in the Iowa medical marijuana program in 2026
Iowa does not call its program medical marijuana. It is the Medical Cannabidiol Program, set up under Iowa Code chapter 124E and run by the Iowa Department of Health and Human Services. The name reflects the program’s origins as a narrow cannabidiol law, although products today can contain THC within a cap. In 2026 the legislature passed House File 990, signed on June 2 2026 and effective July 1 2026, which changes rules on registration cards and dispensary licensure. This post explains HF 990, the rest of the program and what patients should check. It is program and access information only, not legal or medical advice.
A short history
Iowa passed a limited cannabidiol possession law in 2014 for intractable epilepsy, with no in state source. In 2017 House File 524 created chapter 124E, allowing licensed manufacturers and dispensaries and expanding conditions. Dispensaries opened in December 2018. In 2020 lawmakers replaced the old THC percentage cap with a THC quantity cap measured in grams over 90 days.

What HF 990 changes
HF 990 updates the statute on medical cannabidiol registration cards and dispensary licensure. It adjusts how registration cards are issued and renewed and how the department licenses dispensaries, with the effective date of July 1 2026. Patients should check the HHS site for any new card steps, such as renewal timing or fees, and for the current dispensary list. Because the details are set in rule as well as statute, confirm with the department before relying on a specific change.
Who can certify
Iowa licensed physicians, physician assistants, advanced registered nurse practitioners and podiatrists can certify a qualifying condition. The patient then applies to HHS for a registration card, which must be renewed on schedule. Caregivers can register.
Qualifying conditions
Chapter 124E lists conditions including cancer with certain symptoms, multiple sclerosis with severe spasms, seizures, AIDS or HIV, Crohn’s disease, ulcerative colitis, ALS, Parkinson’s disease, untreatable pain, terminal illness with a short life expectancy, post traumatic stress disorder, autism with self injurious or aggressive behavior and severe intractable pediatric autism, among others added by the Medical Cannabidiol Board. These are eligibility categories, and the certifying provider decides.
What did not change
Smoking and flower remain banned. Home grow remains illegal. The THC cap still applies unless a provider certifies an exception. Adult use is illegal.
Federal and hemp context
The DOJ final rule effective April 28 2026 placed marijuana under a state medical license in Schedule III, with a broader hearing that began June 29 2026. Iowa’s program has been described as medical cannabidiol, so how federal changes apply to its products is a question for the department. Federal hemp changes arrive Nov 12 2026 and Dec 11 2026. Iowa also tightened its own consumable hemp law in 2024.

What to watch
Watch department rules carrying out HF 990, board recommendations for new conditions and 2027 bills. Our legal requirements for growing weed plants in your state compares other states.
Getting a registration card
A patient must be an Iowa resident with a qualifying condition certified by an eligible provider. The patient submits the certification and application to HHS, with a fee, and receives a card if approved. A primary caregiver can register to buy and carry products for a patient. Cards are valid for a set term. HF 990 may change renewal steps or terms, so check the HHS site before your renewal date.
The 90 day THC cap explained
Iowa measures supply as total THC over a rolling 90 day period, not by weight of product. A patient can buy different product forms as long as the combined THC stays under the cap. Dispensaries track purchases in a state system. A provider can certify an exception for patients with certain conditions, such as terminal illness, allowing a higher total. Ask your dispensary to show your remaining balance.
Dispensary locations
Iowa started with five dispensaries and has added locations over time, though access remains limited in rural parts of the state. Dispensary licensure changes under HF 990 may affect how new locations are approved, so watch the department for announcements. Some patients drive long distances, so plan purchases around your 90 day window.

The Medical Cannabidiol Board
A board of practitioners and others reviews petitions to add conditions and makes recommendations on forms and limits. Board recommendations can lead to rule or statute changes. Its meeting agendas are public, and patients can submit comments.
Employment and travel
Iowa does not require employers to allow use, and employers may test and discipline under their own policies. Federal workplaces and commercial drivers face federal rules. Taking products across state lines is a federal issue.
Products and testing
Licensed manufacturers produce all products in Iowa, and each batch is tested by a state lab for content and contaminants before sale. Labels show THC and CBD content in milligrams and a batch number for recalls. Keep products in their original packaging with your card.
Program access, age and the law
This guide covers how the Iowa program works and who can access it. It does not describe what cannabis does for any condition, and it is not a substitute for a conversation with a licensed practitioner. Rules can change after a legislative session or a new agency rule, so confirm every detail with the Iowa HHS Office of Medical Cannabidiol before you act. Federal law still treats cannabis outside a state medical license as a controlled substance, and seeds have their own federal rules, so read our December 11 2026 seed rule explainer before ordering. Kind Seed Co sells only to adults 21 and older, and buying seeds is never a substitute for following the home grow rules in your state.
